AMENDMENT TWO (2) TO COMBINED SYNOPSIS/SOLICITATION This amendment is issued to provide responses to a vendor inquiry. The Government received an inquiry regarding Pharmacy services for the Yakama Service Unit, located in Toppenish, WA. Acknowledgment Requirement: Respondents must confirm in their quote submission that they have reviewed and will comply with all information, requirements, and conditions provided in this amendment. Failure to acknowledge this amendment may result in the quote being deemed non-responsive. The following is a list of question(s) received in regards to subject solicitation with responses: Does the solicitation require a candidate to be presented, if so, requesting copies of required form listed within the solicitation. Response: The Government will work with Awardee when the Purchase Order is executed to complete required documents. All other terms and conditions remain unchanged. AMENDMENT TO COMBINED SYNOPSIS/SOLICITATION This amendment is issued to revise the solicitation closing date. Because the original closing date falls on a Federal holiday, the closing date is hereby extended to the next business day. The purpose of this requirement is to provide supplemental professional pharmacy coverage to ensure continuity of pharmacy operations, medication dispensing, and patient care services. The contracted pharmacist shall support the delivery of safe, effective, and compliant pharmacy services while assisting with ongoing clinical and operational pharmacy functions. The period of performance shall consist of one (1) base year and two (2) additional option years, exercisable at the Government’s discretion, to ensure continued pharmacy support services as needed.
